Offsets of ranges which have a CDATASection/Comment/ProcessingInstruction start
and/or end container are not being updated when text is inserted, deleted or
replaced.  This is currently only working for text nodes.  See section 2.12 [1]
of the Document Object Model (DOM) Level 2 Traversal and Range Specification.
